A new way to contact your practice – further information

From Tuesday 22 September 2026, Hammersmith & Fulham Centres for Health will introduce a new way for patients to contact the practice online.

Patients will be able to submit requests for:

  • Urgent health concerns requiring same-day clinical assessment
  • Routine health concerns and medical queries
  • Routine nurse appointment requests, including treatments, procedures and reviews
  • Repeat prescription requests
  • Administrative requests, including letters, forms and general enquiries

Urgent health concerns

If you have an urgent health concern that needs attention on the same day, you can submit a request through:

  • The NHS App – use the link at the bottom of this page for information on how to download the NHS App
  • The practice website

The information you provide will be reviewed by a clinician, who will determine the most appropriate next step based on your clinical needs.

This may include:

  • A face-to-face appointment
  • A telephone consultation
  • Clinical advice
  • Treatment or a prescription
  • Referral or signposting to another service

Providing clear information about your symptoms helps us ensure patients are prioritised according to clinical need and directed to the most appropriate member of the healthcare team.

Routine health concerns, nurse appointments and administrative requests

Patients can use the practice website to submit:

  • Routine health concerns and medical queries
  • Requests for routine nurse appointments, treatments, procedures and reviews
  • Repeat prescription requests
  • Administrative requests and general enquiries

Providing as much information as possible will help us direct your request to the most appropriate member of our team and ensure you receive the right care and support.

Temporary change to appointment booking

As we prepare to launch the new service, routine appointments taking place after Monday 21 September 2026 cannot currently be booked.

From the morning of Tuesday 22 September 2026, requests for routine care and nurse appointments can be submitted through the practice website.

Urgent health concerns will continue to be managed as normal during the transition period. Please telephone the practice if you require same-day medical advice or assessment.

We apologise for any inconvenience and thank you for your patience while we introduce this improvement.

Support for patients who cannot use online services

We recognise that not everyone is able to use online services.

If you are unable to submit a request online, please contact the practice by telephone and a member of our team will assist you. Where appropriate, a member of staff can complete the request on your behalf so that it is reviewed through the same process.

Patients who are able to use the online service are encouraged to do so, helping to keep telephone lines available for those who need additional support.

Medical emergencies

The online forms should not be used for life-threatening emergencies.

If you have a life-threatening emergency, call 999 immediately.

If you need urgent medical help when the practice is closed, visit NHS 111 online or call 111.

Thank you for your support as we introduce this new way to contact your practice.
